Intolerancija nije imunološka reakcija organizma

 
 
Intolerancija, točnije - netolerancija ili nepodnošljivost hrane postaje sve veći i sve češći problem gotovo podjednako u humanoj i veterinarskoj medicini pa mnogi stručnjaci već govore o pravoj epidemiji. Mnogi ljudi često ne razlikuju pojam 'intolerancija' na hranu od pojma 'alergija' na hranu. Naime, alergija na hranu nije isto što i intolerancija na hranu, iako su simptomi često vrlo slični i mogu se „preklapati.“ Međutim, intolerancija ili nepodnošljivost hrane nije imunološka reakcija organizma pa stoga imuni sustav ne sudjeluje u tome. Iz navedenog razloga, intolerancija na hranu nikada ne će izazvati ozbiljnu reakciju poznatu kao anafilaksa ili anafilaktički šok koji može biti i koban za pacijenta. Alergijski testovi na hranu nikada ne otkrivaju postojanje intolerancije na hranu. Ipak, liječnici najčešće prvo provode alergijske testove na hranu.

Alergije na hranu su u stalnom porastu diljem svijeta, a gotovo 60 % alergija javlja se već u prvoj godini djetetovog života. Alergija je snažna i burna reakcija organizma, točnije imunog sustava na bjelančevinu/bjelančevine (proteine). Ove bjelančevine najčešće potječu iz hrane, peluda, kućne prašine, životinjske dlake ili plijesni. To su alergeni koji uzrokuju reakciju ili odgovor imunog sustava na tvar koju organizam smatra toksičnom iako se radi o neškodljivoj supstanci. Nasuprot tome, intolerancija na hranu je KEMIJSKA REAKCIJA koja se javlja kod ljudi ali i kod životinja (mnogo češće kod kućnih ljubimaca) nakon konzumiranja određene vrste hrane ili pića. Intolerancija na hranu danas se povezuje često s astmom, sindromom kroničnog umora i sindromom iritabilnog crijeva.
 
Nadalje, simptomi alergije na hranu javljaju se vrlo brzo nakon konzumiranja određene vrste hrane koja djeluje kao alergen, dok se simptomi intolerancije na hranu najčešće javljaju 12 do 24 sata ili čak dva dana nakon konzumiranja hrane, a to često ovisi i o samoj količini hrane. Naime, u određenim slučajevima mala količina hrane ne će uzrokovati intoleranciju, ali veći unos hrane hoće. Simptomi intolerancije na hranu najčešće uključuju slijedeće simptome: nervozu, tremor/drhtanje, znojenje, palpitacije/ 'drhtanje' srca, ubrzano disanje, glavobolje, migrene, proljev, osjećaj žarenja na koži, osjećaj napetosti u području lica i grudnog koša, simptomi slični astmi te reakcije nalik alergiji.
 
NAJČEŠĆI UZROČNICI INTOLERANCIJE NA HRANU JESU:
Mlijeko i mliječni proizvodi, jogurt, sirevi, čokolada, jaja (osobito bjelanjak), poboljšivači/pojačivači okusa – vrlo često mononatrijev glutamat (poznatiji kao MSG), brojni prehrambeni aditivi, jagode, agrumi ili citrusi, rajčica, vino, osobito crno vino (često zbog sadržaja sulfita) te histamini i drugi amini iz određene vrste hrane.
Dr. Art Ayers (Ph.D.) je znanstvenik čije područje rada se odnosi na molekularnu, staničnu i razvojnu biologiju. Većina njegovih znanstvenih istraživanja odnosi se upravo na crijeva i upalna stanja u crijevima, a intolerancija na hranu njegova je uža specijalnost.
Dr. Ayers na temelju provedenih znanstvenih istraživanja tvrdi da se intolerancija na hranu prije temelji na nedostatku ili odsutnosti dobrih crijevnih bakterija nego na nedostatku enzima, npr. laktaze, ili promjene u imunom sustavu. Većina slučajeva intolerancije na hranu može se pozitivno riješiti ili izliječiti promjenom sastava bakterija u crijevnoj flori. Provedene studije pokazale su da se simptomi intolerancije na laktozu (mliječni šećer) značajno ublaže nakon dodatnog uzimanja klasičnih fermentiranih mliječnih proizvoda kao što su kiselo mlijeko, jogurt i kefir ili dodatnim uzimanjem kvalitetnih probiotika. Naime, bakterije u ovim proizvodima sadrže probavne enzime za laktozu i kada one dobiju priliku da se množe u crijevu, one će svoje gene prenijeti na ostale dobre crijevne bakterije. Ključ je postojanost, stoga simptomi intolerancije su prisutni tako dugo dok se crijevna flora ne popravi u korist dobrih bakterija.

Ugljikohidrati koji se nalaze u cjelovitom zrnu žitarica, povrću, mahunarkama itd. fermentiraju u prisutnosti crijevne flore, ali kada je ona poremećena i kada naši mikroorganizmi nemaju gene/enzime potrebne za potpunu probavu hrane, tada će se pojaviti intolerancija na hranu. Prirodno, povrće je „kontaminirano“ s dvije vrste bakterija – Clostridium i Bacillus koje su od vitalnog značaja za pravilnu probavu i pravilan razvoj imunog sustava. No, kada mi tlo i biljke, tj. poljoprivredne i vrtlarske kulture obilno tretiramo umjetnim kemijskim tvarima tada uzrokujemo oštećenje krhkog ekosustava, a s pretjeranim pranjem i kuhanjem uklanjamo ove važne mikroorganizme. Stanje dodatno pogoršava primjena antibiotika i drugih farmakoloških sredstava, loša prehrana, pretjerana higijena i dr. jer doprinose brzom porastu broja ljudi koji trpe zbog intolerancije na hranu. 
 
U većini slučajeva medicinski stručnjaci preporučuju ili točnije zabranjuju konzumiranje određene vrste hrane za koju je testom intolerancije utvrđeno da postoji visoka intolerancija. Međutim, izbjegavanje takve hrane dovesti će do ublažavanja simptoma ali neće nikako riješiti problem. Dr. Ayers tvrdi da se problematična hrana koja uzrokuje intoleranciju mora izbjegavati sve dok se ne POPRAVI CRIJEVNA FLORA i ne uspostavi zdrava ravnoteža. Tek tada se može vrlo polagano i postepeno ova hrana ponovno uvoditi u prehranu. Međutim, potrebno je naglasiti da to ne vrijedi za intoleranciju na gluten, osobito onu s celijakijom. (Napomena: o celijakiji i intoleranciji na gluten napisala sam nekoliko tekstova koji su objavljeni na portalu HF-a).
 
Dr. Art Ayers preporučuje različite izvore dobrih, korisnih bakterija za uspostavu ravnoteže crijevne flore u razdoblju 'liječenja' intolerancije na hranu. Dr. Ayers preporučuje prirodno fermentiranu hranu i visokokvalitetne probiotike te konzumiranje povrća iz organskog ili eko sustava sa što manje termičke obrade, a od ključnog značaja u terapiji intolerancije na hranu su PREBIOTICI,  tzv. topiva vlakna koja služe kao hrana za dobre bakterije u crijevnoj flori. Prebiotici su vrlo specifična vrsta hrane i predstavljaju zdravi, neprobavljivi sastojak hrane. Oni su otporni na toplinu i stoga su bitni za zdravu 'biokulturu' našeg crijeva. Prebiotici stimuliraju rast zdravih crijevnih bakterija kao što su Bifidobacteria i Lactobacilli te sprječavaju rast i širenje štetnih, patogenih mikroorganizama. Zdrave bakterije u crijevu sposobne su boriti se protiv štetnih i patogenih bakterija u crijevu te tako našem organizmu osiguravaju veliku dobrobit i zdrav imuni sustav. Prebiotici su dijetalna vlakna koja potiču rast dobrih bakterija u crijevnoj flori. Najpoznatiji prebiotik je INULIN, topivo dijetalno vlakno koje se nalazi u različitim vrstama povrća: češnjak, luk, artičoka, šparoga i poriluk su izvrstan izvor inulina a slijede ih zelje, grah, grašak, leća, korjenasto povrće (cikla, mrkva, repa i dr.), kelj, brokula, tikvice, banane, jabuke i jabučni ocat (najbolji je domaći).
 
No, prebiotici se također dodaju i u neke prehrambene proizvode kao što su žitarice za doručak, keksi, kruh i peciva, u jogurte, napitke itd. Stručnjaci svjetski poznate Klinike MAYO ovdje ubrajaju i MED jer on djeluje kao prebiotik te utječe na povećanje razine dobrih bakterija u crijevnoj flori, osobito 'bifidobakteria.' To su potvrdili i znanstvenici 'Michigan State University' čije istraživanje je dokazalo da Bifidobacterium spp izvrsno rastu zahvaljujući medu. Stručnjaci klinike MAYO ističu da su prebiotici neprobavljivi ugljikohidrati koji u kombinaciji s probioticima stvaraju određenu simbiozu, a sami prebiotici su 'gorivo' za dobre crijevne bakterije. Probiotici se nalaze u fermentiranim mliječnim proizvodima kao što su kiselo mlijeko, jogurt i kefir i oni sadrže ŽIVE bakterije korisne za dobru crijevnu floru (voćni jogurti ovdje su izuzeti radi sadržaja šećera ili sladila).
 
Kada se govori o intoleranciji na hranu, tada se najšešće spominje intolerancija na laktozu, a moguća je i intolerancija na kazein (mliječni protein) koja je rijetka i teško ju možemo dokazati. No, vrlo često se koristi pojam „intolerancija na mlijeko.“ Laktaza je enzim koji razgrađuje mliječni šećer laktozu na monosaharide – glukozu i galaktozu neposredno prije probave. Intolerancija na laktozu javlja se radi nedostatka enzima LAKTAZE zbog čega odrasle osobe ne mogu probaviti laktozu. Enzim laktaza stvara se u tankom crijevu; dobre crijevne bakterije, osobito Lactobacillus proizvode laktazu koja omogućuje cijepanje disaharida laktoze na monosaharide i tako omogućuje njihovu apsorpciju. Stručnjaci klinike MAYO tvrde da stanje uzrokovano nemogučnošću probave laktoze manje-više nije štetno ako nije povezano s celijakijom, tj. bezopasno je za zdravlje iako simptomi intolerancije na laktozu mogu biti vrlo neugodni. Simptomi intolerancije na laktozu najčešće se javljaju 30 minuta do 2 sata nakon konzumiranja proizvoda koji sadrže laktozu. Najčešći simptomi su: proljev, nauzeja/mučnina, ponekad i povraćanje, grčevi u trbuhu, nadutost i stvaranje plinova u crijevu. Naime, kod nedostatka enzima laktaze, laktoza iz hrane dolazi u debelo crijevo gdje bakterije crijevne flore dolaze u 'kontakt' s nerazgrađenom laktozom radi čega nastaju simptomi intolerancije. Postoje tri tipa intolerancije na laktozu. Primarna intolerancija: novorođenčad i mala djeca normalno proizvode dovoljno enzima laktaze, a kada počne konzumiranje druge hrane tada se obroci mlijeka smanjuju ili čak u potpunosti ukidaju, čime se ujedno smanjuje stvaranje enzima laktaze u tankom crijevu. Primarna intolerancija na laktozu određena je i genetski i najvećim dijelom javlja se kod odraslih osoba u Africi, Aziji, kod Latinoamerikanaca te kod ljudi s područja Sredozemlja i jugoistočne Europe.

Sekundarna intolerancija na laktozu javlja se kada se u tankom crijevu zbog neke određene bolesti znatno smanjuje stvaranje enzima laktaze (npr. Chronova bolest, ozljede, kirurški zahvati), dok je celijakija najčešći sekundarni uzročnik intolerancije na laktozu. Tercijarna intolerancija na laktozu je kongenitalna: to je moguća pojava iako rijetka, a dijete je rođeno s intolerancijom na laktozu zbog potpunog nedostatka enzima laktaze. Prijevremeno rođena djeca također mogu imati intoleranciju na laktozu zbog nedostatne razine enzima laktaze u tankom crijevu. Intolerancija na hranu određuje se laboratorijskim testovima gdje se u krvnom serumu određuje patološka prisutnost specifičnih imunoglobulinskih antitijela - IgG, a postoje testovi intolerancije na 22, 90, 180 i 270 namirnica, dok pojedini privatni laboratoriji vrše testiranje na 349 različitih namirnica.
 
Dobiveni rezultati intolerancije na hranu omogućuju primjenu tzv. eliminacijske dijete odn. eliminaciju svih namirnica za koje je utvrđen visok stupanj intolerancije. Međutim, stručnjaci upozoravaju na oprez. Naime, istraživanja su također pokazala da simptomi intolerancije na hranu često mogu biti povezani i s nekim drugim poremećajem.

 

Arnold BERSTAD sa suradnicima u bolnici u Oslu (Oslo Lovinseberg Diaconale Hospital) proveo je ispitivanje kod 84 pacijenata koji su imali određene probavne poremećaje i simptome intolerancije na hranu: „Nismo pronašli celijakiju niti intoleranciju na laktozu. Alergološki testovi na hranu bili su negativni,“ izjavio je Berstad u časopisu „Scandinavian Journal of Gastroenterology.“ Istraživanje je međutim otkrilo da su svi ispitanici/pacijenti osim jednog imali sindrom iritabilnog crijeva, dok je njih 85 % imalo sindrom kroničnog umora a 71 % simptome fibromijalgije. Tekst je napisan na zamolbu čitateljice Elizabete i nadam se da će njoj kao i drugima pomoći u rasvjetljavanju ove sve raširenije zdravstvene problematike.

Kojeg li nadmudrivanja u emisiji Otvoreno HRT-a (17. lipnja 2015.) između čuvenih kompilatora i reformatora po pitanju državne mature, profesora Budaka i Bezinovića. Bilo je to pitanje svih pitanja: je li državna matura u stanju zamijeniti klasifikacijske ispite na fakultetima i je li to uopće njen smisao i svrha. Prosječni gledatelj se morao čuditi, jer ono što je samo po sebi razumljivo, a u razvijenom svijetu davno i provedeno, maturante i buduće brucoše nije potrebno dva puta u kratkom roku ispitivati isto gradivo koje su trebali savladati protekle četiri godine. Spomenuti dvojac je polemizirao navodeći sve moguće i nemoguće kvazi argumente za i protiv.

U čemu je 'kvaka broj 21'? Ključno pitanje je žele li uopće naša sveučilišta državnu maturu? Sveučilišta? Pitanje je na mjestu jer npr. Sveučilište u Zagrebu zapravo i nije sveučilište u zapadno europskom smislu. Lijepo je to naznačio i dr. Josef Lange državni tajnik Ministarstva znanosti i kulture savezne države Donje Saksonije na Konferenciji o obrazovanju održanoj 12. svibnja o.g. Spomenuo je da pravna osobnost fakulteta je u suprotnosti s modernim funkcioniranjem sveučilišta kao takvog. Zagrebačko svučilište je skup 'samoupravnih feuda', a ne svučilište kao cjelina.
 
Kad bi 'svučilište' bilo svučilište onda dvojbi oko državne mature ne bi bilo. Fakulteti se s jedne strane ne žele miješati u politiku i rad državne uprave, tj. rad Ministarstva u smislu priprema i zajedničkog organiziranja državne mature, ali im je neobično važno da to Ministarstvo na njihove žiro račune uplaćuje točno utvrđene svotice. Također su naučeni od ranije da im klasifikacijski ispiti nad budućim brucošima pružaju određenu društvenu moć i važnost. Pri tom se pozivaju, gle čuda, na autonomiju sveučilišta, svučilišta koje zapravo u svojoj suštini i ne postoji. Ali zato postoji samo realna autonomija fakulteta! Kad im se stavi ta činjenica kao prigovor najčešći je odgovor da je to naša tradicija visokog obrazovanja. A zapravo to je tradicija samoupravnih OOUR-ova! A što su posljedice?
 
Da su zbog takvog stanja posljedice puno dublje no što je samo pitanje državne mature i dvojbe oko nje, jasno je kad se spozna surova stvarnost, a ta je da ono nije po kvaliteti ni u prvih petsto svjetskih sveučilišta! Interdisciplinarno studiranje je gotovo onemogućeno-nula bodova. Podizanje kvalitete nastave i znanstvenog rada također! Malo ambiciozniji studenti željni interdisciplinarnosti čak i unutar jednog fakulteta doživljavaju se kao elementarna nepogoda. Nedovoljan je broj kvalitetnih profesora i asistenata itd. Naša sveučilišta su HNL, a ne reprezentacija koja može igrati utakmice na svjetskim prvenstvima. Nemojmo se tješiti s ponekim pojedincem poput 'Luke Modrića'. Takav bi svoj vrhunac kao prirodni talent postigao i bez našeg sustava naobrazbe.

A sve je u kriterijima koji su davno srušeni. Sada već davne 1993. godine svjetlo dana je ugledao Zakon o visokoj naobrazbi kojim se željelo postupno organizirati sveučilište po uzoru na europska i po strukturi i po kriterijima za napredovanja u znanstvenim i nastavnim zvanjima. Otpor je bio velik jer su mnogi mogli izgubiti nezasluženo stečene privilegije. 'Turci se dosjetiše' i u izmjenama spomenutog Zakona godine 1996. u članku 51. veznik 'i' vrlo perfidno zamijeniše s veznikom 'ili'.

osoba koja je stekla akademski stupanj magistra znanosti, objavila najmanje jedan znanstveni rad u časopisima s priznatom međunarodnom recenzijom i najmanje godinu dana sudjelovala u istraživačkom radu visokog učilišta ili znanstvenoistraživačkog instituta;
a 1996. godine
b. Izradbi i obrani disertacije izvan doktorskog studija može pristupiti osoba koja je stekla akademski stupanj magistra znanosti i objavila najmanje jedan znanstveni rad u časopisima s priznatom međunarodnom recenzijom ili najmanje godinu dana sudjelovala u istraživačkom radu visokog učilišta ili znanstvenoistraživačkog instituta.
 
Bio je to kraj težnji za međunarodnim kriterijima u znanosti pa tako i u visokoj naobrazbi, a stjecanje doktorata omogućeno je i bez ijednog objavljenog rada s međunarodnom recenzijom, dok su kriteriji srozani na razinu pravilnika na pojedinim fakultetima. Moramo li se tome čuditi kada za rektora Svučilišta biramo osobu koja je bila samo prosječni student FER-a, a na Filozofskom fakultetu postavlja se za dekana profesor pedagogije koji nema niti jednog znanstvenog rada CC ranga. Zato nije čudno da nam u Lijepoj našoj niču sveučilišta kao gljive poslije kiše, a burza rada drži vrata širom otvorena!

Važnost Međunarodne znanstvene koferencije ASMOSIA XI.

 
 
Grad Split je od 18. do 22. svibnja 2015. bio domaćinom međunarodne znanstvene konferencije o antičkom kamenu ASMOSIA XI. Događaj je to koji je u gradu pod Marjanom okupio gotovo dvjesto istaknutih znanstvenika s područja arheologije, povijesti umjetnosti, arhitekture, geologije, kemije, fizike te restauracije i konzervacije kamena iz dvadeset i osam zemalja s pet kontinenata.

Na konferenciji u organizaciji Fakulteta građevinarstva, arhitekture i geodezije te Umjetničke akademije Sveučilišta u Splitu, tijekom pet dana u izvrsno opremljenoj dvorani kinoteke Zlatna vrata sudionici su imali priliku poslušati devedeset predavanja tematski podijeljenih u sedam sesija (geologija i eksploatacija kamena u kamenolomima, obrada i ugradnja kamena, porijeklo kamena, pigmenti na kamenu, napredne tehnologije utvrđivanja porijekla i baze podataka), a u Muzeju grada Splita bila su izložena sedamdeset i tri postera, podijeljena u dvije sesije. U posebnoj tematskoj sesiji, koja je na programu bila u utorak, prvi su put sustavno prezentirane tehnike i tradicija vađenja kamena, kvaliteta i vrsta vapnenca u antičkoj Dalmaciji i Istri, izvoz kamenih proizvoda u Mediteranski bazen te saznanja o korištenju uvoznih vrsta mramora na istom prostoru u antici i njihova reutilizacija u kasnijim povijesnim razdobljima.
 
Organizacijski odbor, na čijem je čelu bila Katja Marasović s Fakulteta građevinarstva, arhitekture i geodezije, okupio je ugledne splitske stručnjake: Nenada Cambija, Tomislava Marasovića, Branka Matulića, Ivu Donellija, Danielu Matetić Poljak, Damira Kliškića, Elviru Šarić Kostić i Vedranu Delongu. U sklopu konferencije organiziran je i izlet na arheološki lokalitet u Saloni, kao i razgledavanje povijesne jezgre Splita s Dioklecijanovom palačom. Po završetku konferencije, u subotu 23. svibnja organiziran je izlet na Brač, u sklopu kojeg su sudionici posjetili antički kamenolom Rasohe, muzej u Škripu, Klesarsku školu i kamenolom „Jadrankamen“ u Pučišćima, a izlet je završen ručkom u Gažulu, gdje su sudionici uživali u autentičnim okusima dalmatinske kuhinje. Također, za ovu priliku priređena je izložba „Marmore laudata Brattia“ autora Ive Donellija, na kojoj su predstavljeni svi vapnenci otoka Brača, kamenolomi, tradicionalni alati i tehnike vađenja kamena, a ujedno je i tiskan dvojezični (hrvatsko-engleski) katalog izložbe, autorica Vinke Marinković i Mione Miliše. Izložba je bila popraćena animiranim filmom o izgradnji Dioklecijanove palače, koji je autor Dalibor Popović tog dana prvi put predstavio javnosti. Donelli je, sa svojim studentima, napravio i poseban suvenir - uzorke dvadeset različitih bračkih kamena, koji su posjetitelji konferencije rado odnijeli svojim domovima.
 
Sva događanja konferencije bila su vezana za uski centar grada i za najistaknutije kulturne ustanove u gradu. U Muzeju grada, osim već spomenutih sesija plakata, održavale su se i pauze za kavu, kao i piće dobrodošlice te završna večera, na kojoj su svirali članovi gypsy jazz benda Repassage. Arheološki muzej, koji su sudionici posjetili nakon izleta u Salonu, bio je domaćinom predstavljanja zbornika ASMOSIA X Proceedings, koji su predstavili Patrizio Pensabene i Eleonora Gasparini s Università La Sapienza di Roma.
 
Međunarodna neprofitna organizacija ASMOSIA (Association for the Study of Marble and Other Stones in Antiquity) osnovana 1988., okuplja više od 250 znanstvenika te ima najveći ugled na svjetskoj razini u proučavanju mramora i drugih vrsta kamena u antici. Povjerivši organizaciju ove konferencije splitskim stručnjacima, ASMOSIA je omogućila Splitu da ugosti svjetski poznate znanstvenike kao što su Patrizio Pensabene, John J. Herrmann, Guntram Koch, John Pollini, Moshe Fischer, Lorenzo Lazzarini, Yannis Maniatis i mnogi drugi.
 
Okupivši najveća imena arheologije i srodnih znanosti, domaćini skupa uspjeli su Split i Hrvatsku prikazati u najboljem svjetlu, o čemu svjedoče brojne pohvale koje su sudionici konferencije iznijeli na završnoj večeri. Mnogi su se složili kako je ova konferencija napravila organizacijski iskorak u odnosnu na prethodnih deset (održanih redom u: Il Ciocco, Italija 1988.; Leuven, Belgija 1990.; Atena, Grčka 1993.; Bordeaux, Francuska 1995.; Boston, SAD 1998.; Venecija, Italija 2000.; Limenas na Thasosu, Grčka 2003.; Aix-en-Provence, Francuska 2006.; Tarragona, Španjolska 2009. i Rim, Italija 2012.) te da je podignula letvicu za sljedeću konferenciju, čije mjesto održavanja još nije određeno.
 
Završetkom konferencije, nije završen i rad Organizacijskog odbora kojemu predstoji veliki posao objavljivanje zbornika radova na engleskom jeziku, čime će se svima zainteresiranima omogućiti uvid u zanimljive i kvalitetne prezentacije koje su, u sklopu konferencije, održane u našem gradu. Tako će po prvi put na jednom mjestu biti objavljeni radovi hrvatskih znanstvenika o antičkim mramorima i drugim vrstama kamenja na prostoru Jadranskog bazena, koji će biti dostupni širokoj svjetskoj znanstvenoj javnosti, što će nesumnjivo doprinijeti razvoj i ugledu hrvatske znanosti.
